2. Healthcare and Wellness Information

Information provided on our website, by email, through text messages, or on social media is intended for general educational and informational purposes. It is not a substitute for individualized medical advice, diagnosis, or treatment.

Services may require a medical evaluation, health-history review, laboratory testing, informed consent, or clearance from an appropriately licensed healthcare professional. Eligibility for any treatment or service is determined individually. We reserve the right to decline or postpone a service when clinically appropriate.

Treatment descriptions are not guarantees of results. Individual experiences and outcomes vary.

3. Emergencies

Our website, email, text messaging, and online booking system should not be used for emergencies. If you believe you are experiencing a medical emergency, call 911 or seek immediate emergency care.

Do not send urgent or highly sensitive medical information by standard text message or unencrypted email.

4. Appointments and Deposits

Appointments may require a deposit before they are confirmed. Unless otherwise stated, the deposit will be applied toward the scheduled service.

We understand that plans can change, so we ask that appointments be canceled or rescheduled at least 24 hours in advance. Deposits are nonrefundable for missed appointments or changes made with less than 24 hours’ notice.

Additional policies may apply to mobile appointments, prescription-related services, laboratory testing, memberships, packages, compounded products, and products prepared or ordered specifically for a client. Applicable terms will be disclosed before purchase whenever required.

5. Mobile Services

Mobile services are subject to provider availability, service-area limitations, travel requirements, clinical eligibility, and any applicable mobile-service charge.

We may reschedule or decline a mobile appointment because of unsafe conditions, severe weather, travel limitations, inability to provide treatment safely, or clinical concerns.

6. Payments, Packages, and Memberships

Prices are subject to change. Payment may be required at booking, before treatment, or when a product is ordered.

Packages, memberships, subscriptions, discounts, and promotional offers are subject to their stated conditions and expiration dates. Unless expressly stated otherwise:

• Promotions cannot be combined.
• Discounts do not apply to other memberships, subscriptions, or packages.
• Services and credits have no cash value.
• Services may not be transferred to another individual.
• Unused benefits may expire at the end of the applicable benefit period.
• Prescription, pharmacy, laboratory, shipping, and dose-related costs may vary.

Additional written terms may be presented during enrollment or checkout.

7. Prescription and Shipped Products

Prescription medications and therapies are available only when clinically appropriate and properly authorized. Submitting a form, scheduling a consultation, or making a payment does not guarantee that a prescription will be issued.

Shipping times are estimates and may be affected by pharmacy processing, inventory, carrier delays, weather, clinical review, or other circumstances outside our control. Requirements and availability may vary by state.

8. SMS Terms

Program description

By expressly opting in, you authorize Mind & Body Transformations to send SMS and MMS messages to the mobile number you provide. Depending on the selections you make, messages may include:

• Appointment confirmations and reminders
• Scheduling and rescheduling information
• Deposit or payment reminders
• Requested follow-up information
• Service and account updates
• Wellness information
• Promotional offers and announcements

Consent

Consent to receive marketing text messages is not a condition of purchasing any product or service. Providing your telephone number without affirmatively selecting the SMS consent option does not constitute consent to receive promotional text messages.

Message frequency and charges

Message frequency varies based on your appointments, requests, account activity, and communication preferences. Message and data rates may apply according to your mobile carrier and plan.

Opting out

You may opt out of text messages at any time by replying STOP. After opting out, you may receive one final message confirming your request. You will no longer receive messages from that SMS program unless you enroll again.

Carriers are not liable for delayed or undelivered messages. Delivery is subject to effective transmission by your wireless carrier and is not guaranteed.

9. Email Communications

If you opt into promotional emails, we may send information about services, wellness education, announcements, and offers. You may unsubscribe through the link included in a marketing email.

Appointment, payment, safety, transactional, or account-related emails may still be sent when reasonably necessary to provide a requested service.

10. User Responsibilities

You agree to provide accurate, current, and complete information. You are responsible for informing us of changes to your contact information, health history, medications, allergies, pregnancy status, medical conditions, or other information that may affect your care.

You may not misuse the website, attempt unauthorized access, submit false information, interfere with its operation, or use its content unlawfully.

11. Intellectual Property

Website text, branding, graphics, logos, photographs, service names, and other original content belong to Mind & Body Transformations or their respective licensors. They may not be copied, republished, distributed, or commercially used without permission.

12. Third-Party Services

Our website may use or link to third-party platforms for scheduling, payments, communications, forms, analytics, pharmacies, laboratories, or other services. Those providers may have separate terms and privacy practices. We are not responsible for the content or independent practices of unaffiliated third-party websites.

13. Disclaimer and Limitation of Liability

To the extent permitted by law, the website and its general informational content are provided without warranties of uninterrupted availability, completeness, or suitability for every purpose.

Mind & Body Transformations will not be responsible for indirect, incidental, special, or consequential losses arising solely from use of the website or reliance on general educational content. Nothing in these Terms limits any rights or remedies that cannot legally be limited.
